Media Release Form

 

I, [Participant's Full Name], give my permission to Ascent Youth Coaching, its representatives, and agents, to use photographs, videos, and other media content of me taken during events, activities, and sessions. These materials may be used on the website www.ascentyouthcoaching.com and other related platforms for promotional and educational purposes.

 

I understand that these materials may be used in different forms of media, such as websites, social media, brochures, presentations, and advertisements. I'm aware that my name, age, and relevant information might be included with the media content.

 

I don't need to see or approve the final product before it's used. I understand that I won't receive any compensation for the use of these materials.

 

I also release Ascent Youth Coaching and its representatives from any claims or causes of action that may arise in connection with the use of these materials.

 

I confirm that I am at least 18 years old and have read and understood this release before signing it.​
